What is the THRIVE Survey?
The THRIVE survey asks about your colorectal cancer, symptoms and side effects, day-to-day activities, experiences with cancer care, and other background information (such as your race, ethnicity, and education).
The follow-up surveys will ask similar questions to the first survey, and will update any information that may have changed.
Some people in the will be asked to fill out a Medical Records Release Form. If you are selected for this part of the study, we will ask if you agree to let us access your health records related to your colorectal cancer care. This will help us learn about the treatment you received. You may choose to complete all or some parts of the study.
If you agree to participate in the study, you will be asked to:
Complete a 30-minute THRIVE survey
Complete a 20-minute follow-up survey 4 months later
Complete a 20-minute follow-up survey 8 months later
Complete a 20-minute follow-up survey 12 months later
Your participation in any part of the study is voluntary. You may choose to complete all or some parts of the study. You may also skip any questions you would not like to answer. However, we encourage you to participate in all components of the study, since that will give a more comprehensive picture of the experiences of individuals with colorectal cancer over time.
Your decision to participate will not affect your current or future medical care in any way. You can choose not to participate or choose to participate in some parts of the study.
Our study team can help you complete the surveys via paper-and-pen, online, or over the phone.

There is no cost to you to participate in this study. To thank you for your time, you will receive:
$40 gift card after you complete the first survey
$15 gift card after you each follow-up survey at 4, 8, and 12 months
$15 gift card after you send back the signed Medical Records Release Form, if you are selected for this part of the study
Gift cards will be mailed to participant upon completion of the survey at each time point.
New Jersey: Cancer is a major health concern that has affected many people in our state. For this reason, the New Jersey Legislature created the NJSCR to help find the causes of and cures for cancer. Every new cancer diagnosis in New Jersey is required by law to be reported to the NJSCR. Information in the cancer registry can only be released for research purposes only to qualified researchers who agree to maintain the confidentiality of the information they collect, with institutional approval. California: We obtained your name and other information about your cancer diagnosis and treatment from the California Cancer Registry (CCR). We did not obtain your name from your health care provider. The California Cancer Registry was created by the California Legislature in the 1980’s in response to public concern that not enough was being done to find the causes and cures of cancer. Every cancer diagnosed in California is required by law to be reported to the California Department of Health Services, which is responsible for the registry. Information on individuals with cancer can only be released for research purposes only to qualified researchers who have obtained approval for the study from a federally approved Committee for the Protection of Human Subjects and have agreed to maintain the confidentiality of the information they collect. No. This is not a clinical study. You are not required to see a doctor or have any medical tests performed. This study does not involve any new medications or changes to your treatment.
All information that you give is confidential and will only be used for research purposes. Your personal details will not be shared with anyone outside the study team (including your doctor or insurance provider). Study reports will never include your name.
Study staff are highly trained to protect your privacy and the confidentiality of the information you share with us. Your privacy is our top priority.
